Chimeric Antigen Receptor (CAR)-T cells have transitioned from an experimental treatment to a mainstream therapeutic with broad adoption. These gene-engineered T cells are revolutionising the treatment of haematological malignancies, and a working understanding of their use, toxicities and limitations is now essential for practitioners in haematology and oncology. Using CAR-T as a prototypical example, this chapter will provide a summary of the current status and emerging role of advanced cellular therapeutics. Focus is placed on the mechanisms underpinning treatment failures and toxicities in CAR-T therapy, as well as the principles of cellular engineering and product manufacturing, which inform emerging strategies to circumvent these limitations.
